I think the number one thing you can do to be safe on a bike is learn to ride a bike well! A helmet and reflective gear will make you more visible and save your head if you crash, but more valuable than all of that is being able to avoid a crash in the first place. Learn to be aware of what's happening behind you, look farther ahead than your front tire, learn to jump a curb if you can't yet, and be able to take a tight turn in case a car corners you in at an intersection. Expect the worst from every car on the road, all the time, and NEVER assume that a car will see you, no matter how much reflective gear you're wearing.

Take as many safety precautions as is reasonable, but at the same time, assume that every single one of them will fail you.

I also recommend shopping around for a helmet until you find one you like. It took me a year, but I finally found a Specialized helmet that looks alright on my head. Don't know the model name, and apparently, they don't produce it anymore.
